What will you do?

 

We are the trusted backbone for families, guiding them through the complexities of aging - in the home or senior living community. Clients and families trust us to provide personalized care that sets us apart:

 

  • Be the Eyes and Ears – Conduct health assessments to understand medical needs, home environments, and available resources. Regularly check in, update plans as needed, and ensure nothing slips through the cracks.

  • Be the Trusted Guide – Anticipate client needs, provide personalized solutions, coordinate care.

  • Be the Compassionate Advocate – Ensure collaboration between healthcare providers and family members. Oversee care quality and compliance.

Who are we looking for?

 

  • Licensed LPN or RN (WI) with strong medical and assessment skills

  • Ability to navigate the healthcare system & advocate for patients

  • Organized, proactive, and able to work independently

  • Exceptional communication skills

  • A passion for making a difference!
